How to use Microsoft Forms

Step 1: Sign In

  1. Go to https://forms.office.com.
  2. Sign in with your Microsoft account (like Outlook, Hotmail, or your work/school account).


Step 2: Create a New Form

  1. Click “New Form” (or “New Quiz” if you want to create a quiz).
  2. A blank form will open. You can now add a title and a description.


Step 3: Add Questions

  1. Click “+ Add New”.
  2. Choose the type of question you want:
    • Choice (multiple choice)
    • Text (short or long answer)
    • Rating (stars or numbers)
    • Date, etc.
  3. Fill in the question and any answer options.
  4. Toggle options like “Required” if you want to make the question mandatory.


Step 4: Preview the Form

  1. Click the “Preview” button (top right).
  2. You’ll see how the form looks to others.
  3. Try filling it out to test it.


Step 5: Share the Form

  1. Click “Share” (top right).
  2. Choose who can respond:
    • Anyone with the link
    • Only people in your organization
  3. Copy the link, or click the email, QR code, or embed options to share it.


Step 6: View Responses

  1. Click the “Responses” tab at the top.
  2. See summary data or click “Open in Excel” to view responses in a spreadsheet.


Tips

  • You can edit your form anytime.
  • Microsoft Forms automatically saves your work.
  • Use “Themes” to change the form’s appearance.
